Clients already enrolled in a Short Term Restorative Care (STRC) Program have the opportunity to enhance their care by collaborating with Skilled Health. By communicating with their STRC coordinator, clients can request a connection with our team to arrange the necessary clinical treatments tailored to their specific needs.
To begin the process of determining eligibility for the STRC program, individuals or their caregivers can reach out to My Aged Care. This initial contact is a crucial step in accessing a wide range of services designed to support older Australians in maintaining their independence and quality of life.
Once the request is made, the Aged Care Assessment Team (ACAT) will conduct a comprehensive assessment. This evaluation is particularly important for those who are encountering difficulties in remaining safely at home due to various factors. These may include general challenges in daily living, recent clinical changes such as infections, or recovery needs following a hospital stay.
The ACAT assessment is a critical component in ensuring that individuals receive the appropriate level of support. It takes into consideration the unique circumstances of each person, including their physical, psychological, and social needs. Based on this assessment, a tailored STRC plan can be developed, outlining the specific services and interventions that will best support the individual in restoring their functional capacity and enhancing their ability to live independently.
By choosing to work with Skilled Health for STRC services, clients can expect a collaborative and holistic approach to their care. Our team of dedicated professionals is committed to providing high-quality, personalised services that address the diverse needs of our clients, helping them achieve their goals and improve their overall well-being.
